Role Overview The Nurse In-Charge plays a critical leadership role in ensuring the seamless delivery of home healthcare services. This position is responsible for supervising nursing operations, managing field staff, coordinating patient care services, maintaining clinical quality standards, and serving as the primary escalation point for both patients and healthcare personnel. The ideal candidate combines strong clinical expertise with leadership, operational coordination, and excellent communication skills to ensure exceptional patient outcomes and family satisfaction. Key Responsibilities Clinical Care Management - Assess patient conditions and healthcare requirements during onboarding and service initiation. - Develop and oversee individualized care plans based on patient needs and physician recommendations. - Monitor patient progress and ensure timely updates to treatment and care plans. - Review clinical reports, nursing notes, and patient documentation regularly. - Provide guidance and support for complex or critical patient cases. - Ensure continuity of care throughout the patient journey. Nursing Team Supervision - Lead and supervise field nurses, caregivers, attendants, and support staff. - Allocate healthcare personnel based on patient requirements, skill levels, and shift schedules. - Conduct onboarding, orientation, and ongoing training programs. - Evaluate staff performance and recommend developmental initiatives. - Address disciplinary issues and maintain professional standards within the team. - Foster a culture of accountability, compassion, and patient-centered care. Operations & Service Coordination - Coordinate with the operations, customer support, and telecalling teams for new patient onboarding and service requests. - Ensure timely deployment of healthcare professionals to patient homes. - Manage shift planning, staff replacements, emergency coverage, and scheduling adjustments. - Maintain effective communication between patients, family members, physicians, and healthcare staff. - Resolve operational challenges to ensure uninterrupted service delivery. Quality Assurance & Compliance - Ensure adherence to clinical protocols, infection control measures, and safety standards. - Conduct periodic patient visits, field audits, and service quality assessments. - Monitor documentation accuracy, including patient records, care reports, vital charts, and progress notes. - Identify areas for quality improvement and implement corrective measures. - Ensure compliance with applicable healthcare regulations, ethical standards, and company policies. Emergency & Escalation Management - Serve as the primary point of contact for clinical escalations and emergency situations. - Guide nursing staff in handling urgent patient care requirements. - Coordinate emergency referrals, hospital transfers, and physician consultations when required. - Ensure timely communication with patient families during critical situations. Inventory & Resource Management - Monitor utilization of medical supplies, consumables, and healthcare equipment. - Coordinate inventory replenishment and availability for ongoing patient care services. - Ensure proper handling, maintenance, and documentation of medical equipment deployed at patient locations. - Minimize wastage and optimize resource utilization.
